Provide direct nursing care support to nurses and CNTs, as appropriate. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures. Meet with your shift on a regularly scheduled basis to assist in identifying and correcting problem areas and to assist in the improvement of services. Ensure that a sufficient number of qualified RNs, LPNs and CNAs are available to ensure quality care is delivered. Make written and oral reports/recommendations to the DON or designee concerning operation of the nursing department. Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Policies and Procedures Manual.
When issues are found, it is the responsibility of the Care Coordinator to work with the Team Leader and their staff to address the issues and train the team working in the home. This is a flex schedule position, all hours including evening, overnight and weekends is to be documented accurately on time sheets and to be turned in weekly. The Care Coordinator s is a supervisory position but will also have to work shifts in their homes if the need arises. Inspect the treatment carts and medication carts/rooms on at least a weekly basis to ensure that they are maintained in a clean, safe and appropriately stocked condition. Report problem areas to the DON or designee and assist in developing corrective action. Inform the DON or designee when physician visits are not made in a timely manner. Encourage attending physicians to sign progress notes, physician’s orders, etc. on a timely basis and according to regulations. Ensure that departmental disciplinary action is administered fairly and in compliance with established disciplinary procedures. Delegate to each Charge Nurse the authority, responsibility and accountability necessary to perform his or her assigned duties.
If kept on as a permanent Care Coordinator , annual job performance evaluations will be conducted thereafter. The Care Coordinator is a supervisory role for Comfort of Home Healthcare and is responsible for the daily operation in the homes that they are assigned. Team Leaders and direct care staff report to the Care Coordinator . My experience of that relationship boils down to one word, “safe”. You’ve got to have the right people around you and I know I do. Participate actively in various committee meetings of the department and/or facility as requested by the DON or designee. Interpret department and facility policies and procedures to personnel, residents, visitors, and government personnel as required. Ensure that a copy of the Nursing Policies and Procedures Manual is available at each nursing station, is current and reflects the day-to-day nursing procedures performed at this facility. The primary purpose of the Shift Supervisor position is to supervise the nursing activities and facility operations to assure resident safety for the whole facility for one shift.
